A number of findings have been made against the Executive by the Ombudsman for Children who looked into the handling of the case.
It found that the HSE failed to allocate a social worker to the child, did not arrange an early face to face meeting with her and failed to acknowledge, consider or address her views which she expressed through her mother.
Emily Logan's office has made ten recommendations to the HSE including continuing to engage with the child and her mother, face to face contact with families and the allocation of a social worker in complex cases.
