Sub-Inspector Negar, 38, who was wounded in the neck by unknown attackers, died early today, Helmand province government spokesman Omar Zawak and police spokesman Fareed Ahmad Obaidi said.
Negar, known by just one name, worked in Helmand police's criminal investigation department in Lashkar Gah city.
She had spoken out about the need for female police officers in Afghanistan, which has seen a string of attacks on prominent women.
Negar's predecessor, the then top woman officer in Helmand, Islam Bibi, was gunned down in July.
Women make up just 1% of police officers in Afghanistan and often face threats from militants as well as their own families.
